When to Start, What to Talk About, and How to Smooth the Transition from Pediatric to Adult Diabetes Care
Helping your patients easily transition from pediatric to adult diabetes care (whether they have type 1 diabetes or type 2 diabetes) can go a long way in helping them manage their diabetes throughout their lives. Several of the major societies, including The Endocrine Society, have released guidelines on transition care, so you know this is a topic you should be paying attention to.
In this video roundtable discussion, diabetes experts, a PCP, and a pediatrician discuss transition care. This video will review tips for when to start the transition process, what's involved in the transition, and what resources you can use to help guide the process.
