Nothing on sestamibi

What happens if you know you have hyperparathyroidism but nothing shows up on your sestamibi so the surgeons do not want to do an operation?
I have calcium level of 11.4 and PTH fluctuates from 30-60. I also have very low Vitamin D and osteoporosis (male and 53). And feel tired and weak all of the time. But, 5 years ago and again this month I have had a sestamibi and ultrasound and there is no noticeable enlargements. Therefore, the surgeons do not want to operate. So, I just keep going to endocrinologists and they just keep giving me blood tests.
What now?
